Merit-based Incentive Payment System (MIPS) Value Pathway (MVP) M0001 is titled “Advancing Cancer Care” and it focuses on the clinical theme of providing fundamental treatment and management of cancer care. Two of the Quality Measures (QM) that may be reported for MVP M0001 are: QM #143 which is titled “Oncology: Medical and Radiation – Pain Intensity Quantified,” and/or QM #144, which is titled “Oncology: Medical and Radiation – Plan of Care for Pain.”
The Description for each QM is provided below:

ICD-10-CM diagnosis code G89.3 [Neoplasm related pain (acute) (chronic)] is assigned to pain documented as being related, associated or due to cancer, primary or secondary malignancy, or tumor; this code is assigned regardless of whether the pain is acute or chronic.
Code G89.3 may be assigned as the principal or first-listed code when the stated reason for the admission/encounter is documented as pain control/pain management; the underlying neoplasm should be reported as an additional diagnosis.
When the reason for the admission/encounter is management of the neoplasm and the pain associated with the neoplasm is also documented, code G89.3 may be assigned as an additional diagnosis.
When reporting ICD-10-CM code G89.3, it is not necessary to assign an additional code for the site of the pain.
CLINICAL SCENARIO:
A 50-year-old female with terminal metastatic breast cancer is admitted to the hospital because of severe neck pain. The pain has worsened over the past week in spite of increasing amounts of oral pain medications. She is admitted for pain management and is initially given p.o. morphine 10-15 mg q. 4-6 hours p.r.n. for severe pain. During the next morning, she continues to complain of severe pain and is given IV (via peripheral vein) morphine 2 mg, which relieves her pain. Eventually, her pain subsides and the morphine is gradually tapered off. She is discharged and referred to the pain clinic for further outpatient care.
FINAL DIAGNOSIS: Severe neck pain due to bone metastasis from malignant neoplasm of right breast.
